Preparing for Your Foot and Ankle Appointment
To ensure your first visit is as smooth and efficient as possible, please bring the following items with you:
- Identification: Please bring a valid photo ID.
- Insurance Information: Make sure to bring your current insurance card(s). We will need to make a copy for our records.
- Referral Information (if applicable): If your insurance requires a referral from your primary care physician, please bring the written referral with you.
- Relevant Medical Records: If you have any previous medical records related to your current foot or ankle condition, such as doctor’s notes, test results, or imaging reports, please bring them with you.
- X-rays or Imaging Studies: If you have any prior X-rays, MRIs, or other imaging studies related to your foot or ankle, please bring the actual images or the reports.
- Medication List: Please bring a current list of all medications you are taking, including prescription drugs, over-the-counter medications, vitamins, and supplements.
- New Patient Form: To save time on the day of your appointment, we encourage you to complete our New Patient Form online prior to your visit. If you are unable to complete it online, please arrive 15-20 minutes early to fill it out in our office.
- Questions and Concerns: Feel free to write down any questions or concerns you have about your foot or ankle condition. This will help you make the most of your consultation with our podiatrist.
Our Cancellation Policy
We understand that unforeseen circumstances may arise, requiring you to reschedule or cancel your appointment. We kindly request that you provide us with 24 hours notice if you need to cancel or reschedule your appointment. This allows us to offer the appointment slot to another patient in need.
Failure to provide adequate notice or a “no-show” for your scheduled appointment may result in a Fee. We appreciate your understanding and cooperation with our cancellation policy.

- Explanation of Your Diagnosis: Once a diagnosis is made, the podiatrist will explain your condition in clear and understandable terms.
